Dental implants represent the most modern and efficient solution for replacing lost teeth.
Whether you lose a tooth, several teeth or all teeth, implants and modern implantology techniques can perform miracles. Thus, patients no longer need to sacrifice natural teeth to anchor dental bridge work types, mobile dentures are no longer used, that creates a permanent discomfort, but instead can benefit from fixed denture, anchored on implants, that looks and feels the same as natural teeth.
Teeth loss represents a real problem, even if their replacement, unfortunately, is frequently postponed or even ignored.
What is a dental implant?
- A dental implant is a pure titanium piece (99.95%), generally in the form of a screw. It has the purpose of creating an artificial root within the maxillary bone, where a single natural tooth or several teeth are missing.
- Reconstruction is made up of two segments – one intraosseous (implant) and one supraosseous called prosthetic abutment.
- Over the prosthetic abutment we can anchor crowns, bridges or dentures.
- The treatment is conservatory compared with a traditional bridge work because dental implants remove the necessity of sacrificing neighboring teeth. In case we manufacture a dental bridge, it requires a minimum of two pole teeth for anchoring, teeth which unfortunately will have to be sacrificed (nerves are extracted, root canals are filled and teeth, once healthy, will be polished).
- Dental restoring on implants brings extraordinary esthetic and functional benefits, patient not being able to perceive the “artificial” sensation. Shortly after insertion, he actually forgets about losing a tooth in that area.
Lack of teeth effects
In the absence of teeth, both mastication and esthetics have to suffer. Additionally, the maxillary bone, where there is no tooth or teeth left, will resorb, namely will melt, so that implant insertion will become risky or even impossible without supplementary procedures, such as bone addition, through which bone volume is recovered so the implant can firmly anchor.
When large groups of teeth or all teeth are lost, facial esthetic has a lot to suffer. Maxillary bones resorb, and facial structures start to modify: lips take on a stifled and thinned aspect, numerous wrinkles appear around the mouth and near the nose and the vertical dimension decreases. In a word, the face takes on a premature aging appearance which represents a great disadvantage.
In addition, mobile prosthetic mastication achieves a maximum of 10% efficiency and many of favorite foods are constantly avoided. The patient, even if is in denial, begins to experience psychosocial problems, develops facial tics, is constantly obsessed with the mobility of the prostheses, avoids smiling and eating, even begins to isolate himself.
When a tooth is lost and it is not replaced, both the neighboring teeth, but also the opposing or opposing teeth will migrate, changing their correct position. This will result in bite imbalances which may worsen by affecting the cheek muscles or the temporomandibular joint. Thus, the patient will experience muscle fatigue and unjustified pain in the head region, as well as a permanent discomfort at dental level and its surroundings.
Migrated teeth will more readily retain bacterial and plaque, which will lead to infections, both at the dental level, through the appearance and complication of caries with canal as well as gingival-periodontal disease by installing and progressing various forms of periodontal disease.
An effective treatment with dental implants is that which aesthetically and functionally restores teeth for at least 30-40 years, meaning, predictable in the long run. This goal can only be achieved by the full involvement of both parties: on the part of the physician, by careful investigation, correct diagnosis and interdisciplinary planning and on the part of the patient by keeping a rigorous oral hygiene at home, compliance with the two annual controls and avoiding or quitting smoking.
Contraindications for dental implant
Dental implant treatment contraindications are few and target metabolic disorders such as uncontrolled diabetes, decompensating disorders, osseous diseases or forms of cancer.
There is no superior age limit because modern dental implants possess a specially treated surface and a design which allows a firm anchoring even into maxillary bones with low density, but there is an inferior age limit, avoiding implant procedures until bone growth phase has ended, meaning adolescence period.
Dental implants cannot be inserted within mouths that present infection or inflammation: active tooth decay, bacterial plaque and scale, and in no circumstances when active forms of periodontal disease or root canal infection are present. Deficient oral hygiene and smoking are the main local factors responsible for implant treatment failure, being major contraindications.
Complete list of conditions and circumstances that can be considered unfavorable (contraindications) for dental implant rehabilitation
Chronic acute and chronic diseases such as:
- diabetes insulin-dependent;
- infection outbreaks;
- polyarticular rheumatism;
- serious conditions of: heart, liver, kidneys, spleen.
Endocrine maladies such as:
- hipo and severe hyperthyroidism;
- overweight obesity.
In psychiatric patients like:
- superanxious (but can be used for general anesthesia);
- very agitated (can be managed with general anesthesia);
Bone system diseases:
- persistent nonspecific infections;
- specific infections like: syphilis, TB, AIDS;
Neurological diseases such as:
- parkinson’s disease;
- multiple sclerosis;
- cerebral palsy;
- trigeminal neuralgia;
- drug intoxications;
- professional manifestation poisoning;
- mental illnesses on the background of chronic alcoholism;
Smokers with more than 10 cigarettes a day;
Allergic illnesses with repeated and serious manifestations;
In some dermatological disorders;
In the patients who have undergone radical treatments with a general condition such as:
- irradiation after tumor removal;
- cytostatic treatment;
- corticoid treatment;
- treatment with anticoagulants for a long time.
The definitive pathological local contraindications are given by:
- jaw or mandible cancer;
- scars post irradiation;
- fibrous osteitis;
- general diseases with headquarters in the jaw.
Temporary local contraindications are given by:
- oral thrush;
- odon lesions (untreated caries);
- periodontal lesions;
- perimaxilar suppositories;
- maxillary sinusitis;
- minor and delimited osteitis processes;
- poor oral hygiene;
- root debris included in bone.
Local anatomical difficulties are given by:
- low alveolar bone height;
- the very small width of the alveolar bone;
- the unfavorable condition of mucoperiost;
- lowered nasal fossils floor;
- lowered sinus floor;
- the unfavorable position of the mandibular canal vertically and vestibulo-orally;
- coronary position of the mesonious bore;
- ratio of neighboring teeth in the root region with the region where the implant is to be inserted.
Patients with endocrine diseases such as uncontrolled or insulin-dependent diabetes, hypothyroidism may have considerable healing problems.
Patients with tuberculosis or sarcoidosis may have a poor response to the healing of surgical lesions. Patients with cardiovascular disease have no healing problems but may have some problems during surgery.
Patients with bone diseases are not candidates for the implant because there is a risk that the implant will fall due to poor osteointegration.
Patients with decompensated haematological diseases with coagulation deficits are surgical contraindications due to hemorrhagic risks.
Systematic smoking is an elimination counter-indication. Numerous statistics show that the success rate of an implant drops sharply at notorious smokers.
Lack of bone volume, resulted as a consequence of postponing tooth replacement after extraction, precisely diagnosed with dental tomography, determines complications in many cases regarding time as well as cost. Therefore, the treatment will be prolonged and with steep prices because manual labor as well as bone restoration materials are expensive, being developed by specialists with experienced and advanced technology. Deficient bone volume is restored through bone addition and augmentation procedures and bone can be repaired and regenerated with biomaterials and advanced techniques.
How is the dental implant treatment conducted?
Dental implant treatment involves a thorough clinical examination, both local and general, elaborate investigations, either blood tests or radiologic investigations, most valuable being represented by dental tomography. On this basis diagnosis can be established which will lead to the proper treatment.
There are several types of works that can be performed on dental implants: mobility works, which patient can remove at home for hygiene purposes and fixed works, cemented or screwed on the implant, able to be detached for control and hygiene purposes only by the doctor.
The dental implant treatment involves several components, three for the main ones represented by the implant itself, with a tronconic or cylindrical shape screw, prosthetic abutment, meaning the connecting element between implant and work and crown or dental work.
Dental works on implants require skilled prosthetists and technicians, to integrate balanced tooth or artificial teeth, in maxillary dynamics as well as modern technologies and materials, biocompatible and esthetic: composites, polymers, porcelain on metallic structure, and porcelain on zirconium structure or full ceramics meaning porcelain without metal, with the most natural aspect. Dental works on implants can be flawlessly manufactured with the help pf dental robots known as cad/cam technology.
The result is rapidly obtained, being precise, esthetic, biocompatible and functional. Dental crown is anchored to the implant by cementing with special cements or by screwing, the latter being a more efficient and comfortable alternative for regular checkups.
For a successful treatment, the dental implant quality used for the treatment is extremely important, needing to be manufactured with quality materials and in special conditions, with certifications and approvals from the authorized institutions in this regard. Last but not least we have to take into consideration the doctor’s qualification (mandatory with field competence, specialization and experience), as well as equipment from the clinic where the surgical procedure will take place (dedicated operating room for this purpose, with all necessary intervention equipment and with total pre and postoperative investigation methods). Being a purely surgical procedure, sterilization techniques will be rigorous and aseptic and antiseptic norms will be rigorously followed.

Missing teeth replacement options
When a tooth is missing, it can be classically replaced through dental bridge, assuming the sacrifice of other healthy teeth or mobile dentures which unfortunately presents numerous disadvantages. Modernly, the lost tooth is replaced with a dental implant, situation in which it is perceived and maintained exactly like a natural tooth (procedure explained in detail here).
When several teeth are missing, replacement is performed with a dental implant for each missing tooth or with an implant for each two missing teeth. Number, distribution and implant position are established only by the specialist, based on objective criteria related to physics and biology (procedure explained in detail here).
When patient has no teeth left, being totally edentulous, a thorough maxillary bone evaluation is required, regarding volume, density as well as position. Overall, posterior maxillary areas are prone to pronounced resorption because molars are generally lost early on. Also in posterior areas, within maxilla, sinuses are found and alveolar nerves within the mandible, anatomic formations that hamper or prevent implant insertion within these regions. When there are no teeth left but maxillary bone retains its volume, inserting 6-8 implants on the arcade allows for a comfortable restoration, either fixed or mobile (procedure explained in detail here). Therefore, in the classical manner, dental implant restoration, when all teeth are missing and maxillary atrophy is present, is performed through adjuvant procedures of osseo-gingival addition and augmentation. But these, unfortunately, prolong treatment duration, considerably raise costs and imply regenerative surgery specific risks.
Teeth in one day!
Alternate methods, which allow total, immediate and fixed restorations without bone addition, are represented by techniques performed for many years with a high success rate at Implantodent: “TotalFix”, “All-on-4”, “Fast and Fixed”. These techniques involve an intelligent exploitation of remaining bone volume, with modern implant aid, axially inserted and angular.
What is important for the patient is that he arrives to the clinic without any teeth or with remaining teeth totally compromised and leaves with implants and works, temporary yet fixed, in only 24 hours!
When all teeth are missing yet maxillary bones are dramatically atrophied, the only and last option which can offer the patient a fixed denture, often immediately fixed is the one where special implants are used, called zygomatic implants. These implants are lengthy and cross through the maxillary bone, anchoring within the zygomatic bones popularly known as cheek bones.
In this chapter, Implantodent specialists own 2 national premieres: “QuadZygoma” as well as “SixZygoma”, meaning immediate and fixed restoration on 4, respectively 6 zygomatic implants simultaneously inserted!
When atrophy is severe, maxillary sinuses volume is enlarged but with bone remaining in the anterior maxillary region, perfect distribution of implants on the arcade is obtained through “TotalFix+” technique, which combines conventional dental implants with zygomatic dental implants. Therefore, along with exceptional support of the work, sinus lifting and sinus augmentation procedures are avoided.
Advantages of dental implants prosthetics:
Dental implant benefits are multiple, some of which are:
- Convenient price – because a single tooth is replaced and not three as in the case of a bridge, a dental implant has a cost close to a traditional metal ceramic work.
- Saves teeth – dental implants don’t sacrifice neighboring teeth integrity as a dental bridge would, because a dental work is supported by implants and not natural teeth. Your healthy teeth are not affected under any circumstance, this being beneficial for your oral health.
- Saves the bone – in the absence of tooth/teeth, bone starts to demineralize (rarify) and to resorb (melt) affecting the mastication system and implicitly the whole body. Dental implant, same as a natural root, positively “stimulates” bone, maintaining its quantitative and qualitative integrity.
- Comfortable – dental implants allow you to smile, talk and eat comfortably and with confidence. Crowns sustained by implants are fixedly anchored and transmit pressures from mastication physiologically into the bone, like a natural tooth, preventing at the same time bone resorption.
- Esthetic – dental works on implants can be unidental (when replacing a single tooth), partial (when replacing a group of teeth) and total (when replacing all teeth). Regardless of the replacement type, dental works on implants seem natural, giving the patient confidence to smile without inhibitions and eat whatever he desires.
- Success rate – successful rate of replacing teeth through dental implants is extremely high (95-97% – mandible and 92-95% – maxilla), if patient presents a proper general and local health status, is not a heavy smoker or excessive alcohol consumer, because a microbial factor may ultimately compromise this achievement.

Dental Implants frequent questions
How long does a dental implant last/resist? Several dental implants successfully fulfill functions even after 25-30 years. Prognosis for patients with periodontal issues in the past, with bone disorders surrounding natural teeth roots, is less optimistic than for healthy patients. A major influence is the way the patient handles all instructions received at the clinic. As every artificial replacement within the body, there are no promises or guarantees regarding implant’s lifespan.
It’s impossible to know how long a certain type of implant will last but by following specialist’s suggestions at Implantodent, the success rate will increase considerably on the long term.
Will the dental implant hurt? Many patients claim that the implant surgical procedure is less problematic then a wisdom tooth extraction. With modern equipment, perfect sterilization, latest generation anesthetics, specialized medical personnel with vast experience in the field, discomfort will become a foreign concept.
Depending on implant type, location and selected type of work, total period of time can range between 3, 6 or more months. There are special dental implants (used in Implantodent clinics) which due to composition and surface treatments can be loaded prosthetic (with teeth) immediate (Nobel Biocare, Straumann).
If bone transplant is necessary, healing period will become longer. We will discuss with you all of these aspects and we will advise you on the recovery period.
Immediately after surgery you will be advised not to wear any king of prosthesis in the operated area. In this short period we will produce temporary works for you to continue life with all its social implications. However, if your social and professional lifestyle doesn’t allow you this, there are modern techniques that can produce an immediate temporary prosthesis (implants which allow immediate prosthesis load – Nobel Biocare, Straumann).
The price for a dental implant is actually the dental implant treatment fee. Total price doesn’t include just the dental implant but also of the prosthetic components: prosthetic abutment and dental crown.
Type and material from which it’s produced dramatically modify treatment price. In another train of thoughts, implant type and quality differ from one producer to another.
Other essential factor with determining role in the final cost equation is the specialist’s experience and reputation as well as equipment and facilities of the respective clinic. We cannot leave out procedures and additional materials necessary in certain cases for restoring altered tissues by resorption, necessary for additions and augmentations, for hard as well as soft tissues. That is why the question “how much will an implant cost?” is totally unsuitable in dental implantology.
Dental implant in Implantodent Clinics
Intensive overspecializations on niche areas of dental medicine as well as advanced technology use have imposed Implantodent as the first clinic in Romania and among the few in Europe where digital implantology is practiced on a large scale. Thus, laser unit synergy, high power microscopes, dental tomography and cad/cam systems from the clinic’s equipment allow fast rehabilitations, minimally invasive and precise with modern implants. In this manner, three dimensional virtual planning on 1:1 implant scale as well as dental work, right before initiating the surgery helps in the surgical guide development, instrument which allows the operator to insert implant in the safest conditions, decreasing the risk of failure close to zero.
“One visit implantology” or “One visit dentistry”
“One visit implantology” and “One visit dentistry”are practical concepts, in our daily activity. Thus, an immediate dental implant is used whenever necessary for rehabilitation in the esthetic area, meaning the front part of the teeth. Even after tooth extraction, in the same session, patient receives an implant, a zirconium prosthetic abutment as well as a dental crown, so he won’t be deprived in any way of continuing his socio-professional activities.
Orthodontics is an important component in our clinic’s portfolio. Thanks to special miniimplants, called orthodontic, our specialists can recover healthy teeth, efficient and non-invasive, often saving them from extraction or prosthetic recovery crown covering.
We have our own anesthesia and special therapy specialist, with pain therapy specialization, classic and with laser. Therefore, besides local anesthesia, we are offering anxious patients or ones with intricate interventions, several other options for anesthesia: inhalosedation, intravenous drug sedation and even general anesthesia through naso-tracheal intubation.
Implantodent clinics include multiple rooms dedicated exclusively to implantology and oral surgery, as well as operating blocks, equipped with everything that is needed for safe and painless interventions under general anesthesia. Postoperative wards are modern and comfortable, so the patients can feel protected and even spoiled, their surgical experience being unique in this domain.
Dental tomography represents the golden standard in dental disorder investigation and diagnosis. We own 5 such computed tomography scanners which offer results in different volumes, from the smallest, when working in isolated areas to ones that comprise the whole cranium when operating zygomatic implants cases or high maxillofacial region disorders. Small radiation dose and images with special resolution recommend three dimensional investigations conducted at Implantodent as being highly valuable for the treatment equation.
Any existing problem and disorder in teeth level, diagnosis as well as treatment options, main and recommended one, as well as alternative ones, are explained in detail and substantiated to the patient, with an adequate language according with his level of medical terms understanding during the specialty consultation session that lasts approximately 2 hours. The cost for this important session, which includes all investigations, including dental tomography is zero for patients who choose to undergo treatments at Implantodent.
Dental implants serve their purpose perfectly, as artificial roots, for many decades, placing within bone being carried out by experienced specialists with advanced technology. Implants still require attention and special care, being properly and constantly cleaned by the patient at home and periodically and professionally by doctors or specialized hygienists in implant cleaning within the office. Smoking and lack of hygiene represent the main enemies of dental implants.
